Provider Application Process and Forms

Notice: Licensing of new foster care homes is suspended by the State of Oregon due to COVID-19. Orientations will not be offered until further notice.

New Foster Provider Orientation
Adult foster provider applicants are required to attend a Washington County DD adult foster care orientation within the year prior to being licensed. Orientations are held in the auditorium at the Public Services Building, 155 N. First Ave., Hillsboro. This location is easily accessible by public transportation and is one block from the Hatfield Government Center MAX stop.


2020 orientation sessions:

  • February 6, 1-4 p.m.
  • May 7, 1-4 p.m. CANCELED DUE TO COVID-19 PROTOCOLS
  • August 5, 1-4 p.m. CANCELED DUE TO COVID-19 PROTOCOLS
  • November 2, 1-4 p.m. CANCELED DUE TO COVID-19 PROTOCOLS 

No registration is required. Participants must attend the entire orientation meeting to receive attendance certificates. Individuals arriving late will not receive attendance certificates.

DD Adult Foster Provider Application Process
The application process is reviewed during foster provider orientation meetings. This checklist may be of use to applicants.

Please note that the application is not complete until all required information is submitted with the required non-refundable fee. Incomplete applications are void after 60 days of the date the application form is received. Failure to provide accurate information may result in the denial of the application.
