County-Provided OHP Services

Washington County Behavioral Health provides specialized care coordination services on behalf of Health Share of Oregon and Trillium Community Health Plan. Care coordination isn’t treatment, it is assistance in accessing needed services and supports. The system of care can be complex and difficult to navigate at times. Care coordinators are experts at the system and work with you to identify what you need to address your behavioral health condition.

While all health plans offer some care coordination, individuals with complex needs or multi-system involvement (such as child welfare or criminal justice) may be referred to the county for additional support. Individuals supported by the county will be assigned a care coordinator. The care coordinator will learn about your needs and desires and will work to connect you with services in the community.
