Intensive Care Coordination (ICC)
While all health plans offer some care coordination to connect members to services, Intensive Care Coordination (ICC) offers additional support to individuals with behavioral health conditions who have complex, multi-system involvement. ICC staff develop individualized plans to connect individuals to needed services and supports in their community. These plans are person-centered and strive to coordinate care across multiple systems such as health care and substance use treatment.
Washington County currently provides ICC services for individuals with behavioral health conditions who are OHP members assigned to Health Share of Oregon.
